I haven't seen any messed-up movies lately, but the most messed up movie I ever saw (because it was real) was Crumb. Robert is an interesting, weird dude, but he was the most "normal" one in the family, everyone else was totally messed up (maybe NSFW clip from the doc).

The Director was murdered during post production of the film by someone who claimed to have stolen film reels, and told the director to meet in an undisclosed area to get the missing reels back in exchange for money. Just adds to the strange lure behind the film.
