Sure, but I would just call the high school coach, principal, etc, and ask questions.
Not send out a form with stupid questions like high school 40 times which aren’t relevant.

In order to fill out the whole form, and do it well, one would have to do a lot of digging, phone calls, secretary and track coach requests, etc. I hope the Packers sent a check for $50 along.

Pretty sure they've done that forever. They check each players background extensively.

frick that. The pro teams have scouts. It’s their job, not some guy who coached him in HS. The only way I’d complete that paperwork is if I got paid handsomely to do so.
