Karthus is one of the safest mid lanes in the game. His Q range makes him impossible to harass if you aren't retarded. He safely farms outside of the range of most assasins. He is weak to burst in lane, but karthus should always be too close to his tower for the enemy assassin to go all in. For good karthus players the enemy laner doesn't even exist, its all farm and map awareness. The obvious exception to all of this is leblanc. She can close the distance hard, silence, and then burst the frick out of karth.

Idk about one of the hardest but yeah you have to hit skill shots at some point. However playing karthus I can farm and not land a single q on my lane opponent and still win lane hard. Karthus honestly shouldn't ever fight his lane opponent unless they are low or have a jungle gank.

I love people who know they have to be somewhere in like 20-25 minutes and still queue up for ranked. We were stomping the game for 25 minutes, all the while Vlad bitching about us needing to go faster because he had to go somewhere. He eventually afks and we slowly get pushed back after having their inhibs at one point. Sigh. Rip 6 game winning streak.

After playing new Karth on the PBE his AA is more responsive but still bottom tier in league. His q is also way more responsive and seems a lot easier to hit.

It does seem strange. Some games I'll get pitted against people who I completely outclass, and others I get my shite pushed in because the enemy team is clearly out of my league.
