I'll add some new ones since this thread brings the usual suspects like Battle toads and Top Gun.
SNES Jurassic Park. Game had no saves so you not only had to not die of course, but keep your system running constantly. My brother ended up dying near the very end due to some confusion. Lame.
I recall Paperboy being a bitch. You had to get all the way through a level and had only so many lives. Pretty sure we only made it a couple levels. One of those early Simpson games worked the same way

My best friend in High School broke his controller then threw it up in a tree after fighting Spectral Keeper in FF10 multiple times one day and losing everytime.

Battletoads (NES) - the hovercraft level
TMNT (NES) - the airport level
Silver Surfer (NES) - there is not a single stage in the entire game that is possible to beat

Marble Madness on XBL was pretty friggin hard. Not the worst, but the higher levels made you pull your hair out.

SkyShark on NES. fricking impossible vertical scroller where shite comes flying at you right out of the gate from 1000 different directions, no continues, barely any lives. frick that game

I've been out of gaming for a while, but thinking back, it's amazing how impossibly hard the games I grew up with were. It wasn't even fun. I remember being a kid and my dad took me to rent a game, but it was so hard that I broke out in tears. I couldn't figure it out to save my life. It was pretty much going to ruin my entire weekend. So my dad felt bad for me and took me back and got a different game.

I was on the final level of Super Ghosts and Goblins when I was inexplicably warped back to the graveyard. I sat there for a moment, rethinking life.
