Once had a roach run across the table on to my plate of food at R&G in Chinatown. At last glance, it scurried under the metal doors into the kitchen. Complaint fell on deaf ears. Never again.

Slanted Door in the Ferry building. Probably need reservations, but you can always try. They also have a take-out location (Out the Door) in the same building. This now has a second location that does dine-in on Bush Street, but that's in the Pacific Heights area.

Went there a couple of weeks ago. Spilled my beer and broke the glass. I was reaching for something and my wife said the button on my jacket sleeve/cuff caught the glass and literally catapulted the glass. Luckily there was only another couple of sips in the glass.
