In instances where an employee is unable to produce
acceptable proof of vaccination under paragraphs (e)(2)(i) - (e)(2)(v), paragraph (e)(2)(vi)
provides that a signed and dated statement by the employee will be acceptable. The
employee’s statement must: (A) attest to their vaccination status (fully vaccinated or
partially vaccinated); (B) attest that they have lost or are otherwise unable to produce
proof required by the standard; and (C) include the following language:
“I declare (or certify, verify, or state) that this statement about my vaccination status is true and
accurate. I understand that knowingly providing false information regarding my
vaccination status on this form may subject me to criminal penalties.”

one of the other forms of vaccination proof for that employee and, depending on the

So you got this via Facebook post to suggest people how to avoid getting vaccinated while keeping their job...and your advice is criminal fraud?

People are going to be trying to explain to their companies that the letter is allowed when companies demand proof.

If I'm reading the tea leaves correctly, you're banking that a company's HR group will agree to attest you're vaccinated but didn't show proof via card. Think they'll put their name on it and submit?

You can provide a signed letter attesting that you are vaccinated as proof, claiming you lost your vax card
