But seriously, that was a question I had from Batman Begins - is there any explanation of how the voice is altered so much when the mask is on?

no. i'm not sure why either, it would have been easy to explain and then the voice thing would be a non-issue

unlike Clark kent who uses only glasses to disguise himself, it appears that the batman over the last 20 years also disguises his voice. Unfortunately bale doesn't pull it off quite like Keaton did.

I have watched this 3 times, now, and I like Batman's fourth question best. This is where he clearly becomes upset that the Joker cannot understand.

The opening scene of Batman '89 was great. I am talking about when Keaton holds the guy by the shirt collar, hanging him over the side of the building, and says, "I AM BATMAN!" That voice is how I think the voice should be

Yeah, Keaton did it good. But overall, Nolan's two Batman movies beat any of the Batman movies from the '80's and '90's(Batman, Batman Returns, Batman Foreverver, ext.)
Bale's voice may be over the top, but it isn't something I dwell on when rating TDK.
