Being close to someone makes you miss of the tell-tale signs of someone abusing pain killers.
Also, if you've never been around anyone that used drugs you may not know what that looks like either.
What are the subtle signs?

Your painkillers are missing from the medicine cabinet.

If they use pain killers at all, they are abusing them.

If their pain subsides, that's a tell-tale sign.

Depends what pain killers, how bad the abuse is, their method of doing the pain killers...
There is way too many questions to give you a definitive answer honestly.
Good chance someone is taking 4-8 lortabs Orally a day and you would have zero clue unless you saw them taking it.

If they throw up after 12 drinks, they are probably on opiates.

Fast talking, pin hole eyes, itchy nose, erratic behavior followed by nodding off..

It depends. Some people hide it better than others, but has this person changed at some point? If they are hiding it, they will lie about where they are going, where they been, what they been doing, etc.. Do they slur at times? Do they sleep a lot?

Abusing is different than being physically addicted, fyi.
Also, there is no for sure way to tell. Opiates effect everyone differently.
Abusing- droopy eyes and have a hard time staying awake. but some people get tons of energy and are hyper while on opiates. Some are constantly scratching all over body.
And honestly, most of the ones who abuse let you know at some point or another. They generally have issues with self control in other areas of their life too.
Addicted- if they are out of their pain pills, they will have watery eyes, will be yawning constantly, will complain about freezing one minute, and sweating the next, will have chills and goosebumps in the first few days, runny nose, and diarrhea.

Whenever I took too many loritabs, I was itching like crazy. Don't know if it was me, or it effects everyone like that. I gotta admit, I was drinking a good bit of beer, too.
