Lol I remember when I saw it in theaters just sitting there for a second after it happened and being like, "Wait, WTF did she do to deserve that, like why...."
It didn't ruin the movie or anything for me, but it definitely didn't really go with the flow, IMO.
Now my worst death on screen, at least where it impacted me most, was G-Baby dying in Hardball. I cry every damn time man.
If we're talking brutal, cringeworthy, the Casino one is definitely up there. One that hasn't been mentioned that I think would suck because of how possible/easy it would be to do is the murdered couple out at the picnic in Zodiac. Two people minding their own business and boom, stabbed non-stop by some random baw.

I just remembered the fire extinguisher death scene in Irreversible. That is probably the most brutal, realistic death scene in a movie. Go watch it if you don't believe me.

I feel like Kane's death in Alien should be by far the most common answer to this question. Arguably the most famous violent/gorey death in movie history.

Not a death scene, but the needle pit scene in Saw II is so painful to watch you can feel those needles...I can't even look at it.
