SCUBA with reef sharks (not as dangerous as it sounds, they actually shied away from us and we had to swim towards them to get a look. But still, I was 40 feet down with freakin SHARKS).

Most of things that I did before age 25 were extreme, including unprotected sex with women I had just met, driving without a seat belt and having no health insurance.

Put a friend on something very similar to this sans that green tower part and pulled it behind another friend's car going about 25. Since it had no steering, at one point the thing swerved and my friend was almost decapitated by a mailbox.
The crotchety neighbor called the cops. When they showed up they laughed, told us it was something they would have done, took pictures of us on it, and asked us not to do it anymore.

I got into motocross at age 34. I had no previous motorcycle experience other than Honda Z50 and Trail 70 as a kid. But I used to go to the track to watch my friends race when we were kids and I was always a fan of the sport. So when I was 34 I bought a used Honda XR200 to ride on the levee, then went full bore and bought a new Yamaha YZ426 and started going to the track and racing. It's a thrill to stare down a 75 foot triple and jump it for the first time, knowing that if you come up just a couple feet too short, bad things will happen. Enjoyed it for several years, though it did take its toll - broke my hand and had to shut down my dental practice for a month, punctured my calf muscle, injured my neck (which was the final nail in the coffin that put me on disability). I also did track days on a street bike and in my Porsche, but nothing makes your ahole pucker like hitting a huge jump for the first time.
ETA: Another thing I've done that I guess qualifies as extreme is catching a venomous snake bare handed. Serious adrenaline rush there, knowing the consequences of the snake being quicker than you.

"Snowboard" down an iced over mountain road... Busted arse and was sore for a few days but we picked up some serious speed and it was fun as the frick.
