potential End of Life medical decisions Questionnaire?

I'm attempting to set up some directives for myself and older family members about their wishes, specifically regarding medical decisions, should any of us become unable to communicate, etc.
Surely there's some info to help point individuals in the direction of decisions that can be made prior to these events.
A quick google search leads to numerous sponsored sites, that for a fee, they will provide you with questions. They may or may not be what I want, and I don't feel like paying to find out.
Any suggestions on somewhere to look for some information regarding choices and decisions to be made?

I would call your choice of a funeral home of your choice and see if they can guide you in the right direction. Maybe a hospice also?
And mentioning a funeral home was not being a smart butt.

My health plan's General Physician has several lists to consider. If you have a regular GP, ask his nurse. Very detailed with full explanations.

There is a workbook called “5 Wishes”. They sell them but you can usually get them from a social worker at a hospital. You can also find pdf copies online. It walks you through decision making.
You should also be able to ask your PCP about info related to this.
Once done and either notarized or consigned by 2 independent witnesses (not family or your provider) make sure you get a copy to your PCP and let family know where your copy is.
Also worth reviewing periodically to make sure your desires have not changed as your circumstances may change over time.

just get and keep your affairs in order and keep a big handful of pills handy. When you feel you can't go on, down 'em. You certainly do not want to ever go into a nursing home.

My wishes: Find the cheapest roaster and take a vacation to a place that means something to us all. You can dump me there or any trashbin along the way.

When you get the form completed make sure you share it with your primary care doctor and local hospital. They cannot comply with something they don’t know about.
