The GLP-1 Protocol

So many of you have asked what i think the protocol for the GLP-1 class of drugs should be since i have made it really clear i do not agree with current guidelines.
The reason I do not care for current guidelines is I feel it lacks the emphasis of lifestyle changes and the use of minimum effective dosing to get use where we want to be.
I also feel like there are ways to minimize the loss of muscle, the sagging skin and overall aging that we see with many on GLP-1 drugs.
To be clear I beleive Mounjaro or now Zepbound should be used fpr fatloss if you are going to go down the road of GLP-1 drugs. It is more effective and has less side effects from all of my research.
I believe that to decrease the chances of staying on the GLP1 drugs for life it is very important to slowly titrate up using minimum effective dosing and then once we reach goal weight, we titrate down dosing in 3 month blocks. The goal is to not have to go to 10mg to reach our goal and to titrate down until we get to the lowest dose of 2.5mg which we will stay on for 6 months before coming off. The ultimate goal is to be at goal physique and not be on the medicine.
-The peptides protocols can no longer be prescribed by a doctor to my knowledge, this is as of a week ago. Talk to your doctor about it. You can still purchase through peptidescience, QSC on mesorx, and limitless life nootropics.
- Sustain alpha can be purchased from iconic formulations. Dermacrine can be used also, along with any of their other products. No need to throw the kitchen sink at things right away though.
- the fat loss protocol can be purchased from SNS or any online retailer that sales SNS products.
The Mounjaro Protocol
Goals:
1) Use lifestyle intervention to provide the majority of the weight loss while minimizing dosing of Mounjaro.
2) Looking to lose 1-2 lbs per week. The 1st four week block we are looking to lose 10lbs.
3) We will take a measurement at the naval and we will use that and the scale to track body fat loss. Getting a Dexa, Bodpod or Inbody Scan would be preferred, but not required.
4) We will eat in a slight caloric deficit and we will prioritize protein to minimize muscle loss and to minimize sagging skin that is so common with those that lose extreme amounts of weight.
5) We will use intermittent fasting and eat all meals in the noon-8pm or 11am-7pm time frame.
6) Males will have testosterone checked. If done having kids and TRT is needed, use. In younger males you will use Sustain Alpha from Iconic Formulations to keep Testosterone levels elevated while we reduce calories. 5 pumps per day.
7) We will progressively layer in other protocols to help with the weight loss as needed.
8) We will lift weights progressively 2-3x per week. I highly suggest starting with just the bar and sticking to a basic linear progression program. I highly suggest the Greyskull templates. We must get strong and build muscle if we are seeking the aesthetic appearance we claim to want. You must build muscle to keep skin from sagging. This is extra important for females.

So where do we go from here………….well we raise the Mounjaro to 5mg and we start the whole process over again. I Do not feel I really need to type this all out but we will raise calories, lower exercise activity and drop the GH/Supplement package back down to the original protocol. We will repeat all of the above steps just like we did being consistent with the diet and the training.
Only after completing the above steps at the 5mg dosing will we move to 7.5 mg dosing. At that point it will look like the following.

ok there you have it, systematic way to properly use GLP-1
now if you have issues with the stimulants, dont use them. pretty simple. It may cause you to plateau quicker but that is ok.
i also understand for many it can look overwhelming...its really not.
it start real simple and slowly titrates calories down, walking up, and slowly ramps up the growth hormone protocols.
we want to stay on the lowest doses we can of all this BTW. just enough to reach physique goals. Wont be able to answer a ton of questions today, but i can tomorrow.

Yes on the iPam for ghrp but the ghrh choice should depend on where you are imo.
Tesamorlin is a secret weapon that should be saved until other things have been exhausted…imo.
The combo of Tesa as the ghrh and higher doses hexarelin for about 6 weeks would be ideal then switch the hex out for ipam with a nightly boom dose for 8-12 weeks and then go back to normal mod gfr 1-29 as the ghrh and normal dosing of ipam with bpc for a while at 2-3x per day.
That’s kind of what I was getting at, just didn’t prescribe the hex because it can only be used 6-8 weeks for your ghrp before it becomes less and less effective.

Is the only real way to fix loose skin by building muscle? I ask because I have a family member who lost a lot of weight and now she's self conscious about her loose skin but there's no way I could see her lifting weights unfortunately

If she already lost it, pretty much and even then only so much can be done. That’s why it’s important to start with high protein, lifting and the gh peptide protocol.
If you are extremely obese you can even start the bpc-157 to help the skin heal
So in her case, I would recommend starting the gh protocol listed above with the bpc 2x per day plus the 3x per day gh peptide protocol or if willing, get on regular growth at 3iu daily but results are going to be very limited now that she already stretched the skin beyond what it was capable of and didn’t keep it moist and didn’t start things from the start.
That’s one of the big draw backs of glp-1 drugs and once it starts hanging, almost too late.
If you are extremely obese you can even start the bpc-157 to help the skin heal

Great post. As someone who used Mounjaro and got great results, I would add a few things in.
1) The 5mg is going to be the highest for most will need unless they are severely obese. The higher the dose, the more side effects were reported.
2) Supplement with Magnesium daily.
3) Collagen supplements for skin
4) Take your shot at night before bed. The first 12 hour are where people reposed nausea. Ask your provider for under the tongue nausea meds.
4b) Take your weekly shot on Wednesday/Thursday. The strongest effects of the drug will be hitting you during the weekend.
5) A diet/lifestyle change greatly increases the effects of the medication. There are people who don’t change diet and did fine, but low carb will help with digestion issues and help get into an even lower caloric deficit. This will also make intermittent fasting much easier.
6) When you eat, eat as slow as possible. By your 4-5th bite of food, you’re not going to want to eat much more. You can eat more if you hork it down, but that’s defeating the purpose in my mind.

*DO NOT ALLOW YOURSELF TO GET INTO TOO STEEP OF A DEFICIT!!*
also it is of the upmost importance, you get a bare minimum of 150g of protein while on this medicine. it will not burn muscle but it will put you in an extreme deficit and extreme protein deficit if you let it.

Thanks for this post. Gone from strong and a little fat to really fat and a little strong over the past 15 yrs.
Is there anyone you recommend that I could work with a coach to implement this? Have always been intimidated by supplements/peptides and would like to talk to an expert that’s helping with implementation and accountability

Sorry, but false.
Some people actually do struggle with weight lose. Even in the 50-80 pound range.
GLP-1s have also reduced the overall rate of major heart problems by 20%.

Please explain exactly why in detail, not I heard or I feel, explain in detail with links please
Because you are wrong by every available bit of data. I would imagine you’re going to cite a side effect that happens in about 1 in every million people. We have over a decade of data showing it’s safety and how effective it is for not just obesity but blood sugar, heart issues etc
