My wife and I are planning a trip this summer. Currently the plan is to fly into Boston, and spend a couple nights to catch a Sox game. Then we'd figure out how to get a rental car, and drive down to Cape and spend 3-4 nights there.
What's the best way to handle the rental car? I think we'll do a BnB in Boston that won't have a parking space, so I'll need to plan to pick up the car when we leave town, then return it before flying out. Any good Cape suggestions?

Where on Cape Cod? Outer Cape is pretty different from Lower and Mid Cape, for example. This is also the difference in car rental versus not. If you plan to spend most of your time in Provincetown, no need to rent. If you plan to traverse Route 6, rental is necessary.
Dad has a place in Truro, which is closer to Provincetown - can definitely help with most anything Outer Cape, but someone (i.e. Abita) may have to be the helping hand for anything else.

That’s where we stayed and we loved it. Def make a point to eat at Skippers Chowder house. Our entire group agreed that was the best meal we had the entire trip which included coastal Maine and Boston. Yarmouth isn’t as central cape as Chatham but we drove the entire cape and there’s so much to enjoy looking at along the way the drive wasn’t a big deal at all. Also you’re right there close to Hyannis where you can ferry out on a day trip to Martha’s Vineyard or Nantucket. (I def recommend this - we did MV ).
As far as rental car goes I assume you could save some $ renting away from the airport but may wanna find one that is walkable to a T station so you could take the T back for your flight (or just Uber ). If you don’t mind paying a little more though renting from the airport might be the way to go so less hassle getting to your return flight.

But it was crowded during the week too. We had planned to go to Skippers for supper but it was a 2 + hour wait.
We took the ferry to Nantucket for a day. We got the ticket on line. If you go there definitely go to the Whaling Museum.
We drove up to Provincetown the next day. We did a dunes tour which took about an hour and was worth it.
Stop by the National Seashore Visitor Centers, there are at least two, and watch the videos. One of them has a small museum. We saw whales at Race Point Beach but all you could really see was their water spout.
If we go back I would look into a whale watching tour.

We enjoyed learning about the history of that area of the Cape from the driver.

Thanks… I think we’re going to stay pretty close to Fenway. Would it be easy to take the train from the airport to where we’re staying and back? Or would Uber work better?

Blue line from airport to green line will get you to Fenway if I remember right. Probably take an hour though.
I'd agree to renting at the airport and just going back to get it when you are ready for it. So much easier on return.

Do not rent a car for the time you’re in Boston. The roads are not easy to navigate and drivers here are not exactly the friendliest and most patient. Plus if you’re driving around the city you’ll have to likely pay to park in a garage which is the same or often more than just paying for an Uber.
Using Uber is the easiest and probably fastest way to get from the airport to Fenway, but the T would be free.
Silver line bus to south station. Red line to park street. Green line to Fenway (or whatever the proper stop would be). It will probably take about 45 minutes to an hour.

I would just take a ferry into P-Town and stay there myself if it was it first time rather than renting a car. You can also pickup a turo rental once you get to P Town if you wanted to explore Chatham, Orleans, etc

Parking in Boston is awful and expensive
Even the hotel parking is a lot

to add to things to do:
Cape Cod League baseball. If you are a fan you can catch up to three games a day
Old Railroad trail they ahve converted to a bike path. Rent bikes and do the trail
Drive inn movie in Wellfleet
Whale Watching
Just a few things that have not already been mentioned.

Agree with a previous reply… you can always take a ferry from Plymouth to Provincetown. Can take the P&B bus to Plymouth very cheap too. Or even commuter rail; Plymouth is a good spot to spend a day too fwiw. Aside from Plymouth Rock, the mayflower, pilgrim stuff, etc. there are good bars, restaurants and shops walkable in the downtown area near the tourist stuff.
My favorite spot on the cape is Chatham which you’d need a car to visit, but I prefer Martha’s Vineyard and Nantucket to anywhere on the cape.

If you and your wife like beer, definitely go to Tree House in Sandwich. It’s a really nice location and the beer is outstanding.
Warning: you have to purchase tickets in advance. You can’t just show up.

For a first time visit to the cape I feel like you are definitely gonna want a car to explore. Provincetown is LGTBQ-ville and while it’s interesting to walk around there for a day there is just so much more to the cape that’ll require a car to see.

Just a heads up, the Fenway Trillium is very small so they don’t have as many offerings as other locations. But since you’re staying near Fenway I know that will be the most convenient. If you can make your way to the Seaport, I’d highly recommend going to that location. However, if you’re at the Fenway location you can pop into Time Out Market and get some really great food from restaurants all over the city.

Depends on the time of year and day of the week. Usually not, but on a weekend during the summer there’s definitely a chance.
Here’s the catch: tickets only become available 2.5 hours before your start time. So you need to be on top of it. Also, your ticket allows you a certain number of pours, and you only have a 3 hour window to be there.
I realize this all sounds like a ridiculously restrictive pain in the arse, but the reason is because the facility is built right on the ocean in a residential neighborhood, so in order to get licensed they had to do a lot to appease the (wealthy) residents of the area. But it’s worth it.

No car for Boston.
Car for the Cape.
I would:
Day trip to Ptown.
Day trip to Chatham.
If you want to do some beach time, Eastham and Wellfleet have some nice Atlantic beaches.
Cahoon Hollow beach during the day and dinner at drinks at Wellfleet beachcomber right above it is a nice day. Limited parking there.
Car for the Cape.
