Recommendation for Cell Phone Carrier

I use my cell phone as my pager. My answering service sends my pages as text messages.
I've been with at&t forever. I've had an occasional issue with a page that I receive hours after it was sent. As a physician, this can be an obvious issue. I've tolerated it since it's occurred only on occasion.
This weekend, it has occurred 5 times. Specifically, a daytime page has pushed through to my phone at 3 am each of the past three nights. I've had it! I'm ready to switch carriers. Who can I trust?

Truly depends on your area and where you plan to use your phone most.
Generally speaking, Verizon offers the best coverage, but is the most expensive. Followed by AT&T, T-Mobile and Sprint.
YMMV.
I personally have used Sprint for over 8 years and while their past reputation is well deserved, today's version of Sprint is unlike years past. It works. And it works everywhere I need it. I'm never without service, text messages go through quickly and 4 lines with unlimited everything for $100 (plus tax and fees) is unbeatable.
Your best bet is to go with the carrier that you think works best for you in your area and try them out. Return the phone and cancel the service before their return policy expires. You're only responsible for the activation fee (if any) and service that you use during your "trial period."

If his primary focus is reliability I think the only place he can go would be Verizon, although I question whether this is an AT&T issue or something else. Anything else would be a downgrade for reliability.
IMO the only reason to go below AT&T and Verizon is price. And I say that as someone who has had Sprint, Verizon and am finishing up a 2 year experiment with Google Fi (Sprint and TMob).

Because being on call for me also entails rounding, admitting patients from ER, etc. Calls inevitably come at the most inopportune times.

Also with AT&T and noticed the delayed- and repeated-text issue this weekend -- text service is normally rock-solid and stupid-fast on my Blackberry Q10. Feels like text delivery time has crept up in the past few weeks.

It is still pretty terrible unless you're next to a tower or in an area in which they have high tower density. All of their wideband LTE is high band spectrum, which doesn't penetrate walls. Unless they densify their network (which they won't since they aren't investing on their network at the moment), Sprint will have islands of super fast LTE+ and oceans of slow LTE. Their high band spectrum has such poor propagation that even natural foliage can block the signal.

Outside of data, Sprint does work. Unfortunately, I did notice that texts would be delayed whenever I was on 3G. Messages that go over LTE are fast, but when they go over 1x they take a few seconds to send and sometimes up to 30 seconds to receive. A few instances, I had messages come almost 30 minutes late.
Voice quality has always been great, but unfortunately there is no simultaneous Voice and Data. Also, all their prices seem to be promotional offers. After promos are over, their service doesn't justify their price.

I was with Cingular from the beginning before it got bought by ATT years ago. About two months ago, ditched ATT and bought an Android phone with TracFone service (which in most cases is Verizon). I couldnt be happier. FUG ATT

I agree completely. I'll be out of Sprint just before my free year is over. Tree fiddy a month is a great deal, but I wouldn't pay a lot more for it. The LTE is frequently abysmally slow in BR. I think George's on Highland must have lead walls. OTOH, go near Mall of Louisana, and you're in LTE heaven.
