Hmmm, I'm going to guess:
- red flags in the performance of the crew
- command climate issues, probably some woke-based discrimination taking place
- numerous reports to congressional members, IG etc
- investigations revealed misdeeds
- coverup of misdeeds
- inappropriate relations
