There are always outliers though so what would you propose for those absolutely incapable of working? Not those who choose to not work, those who've been injured so badly they cannot return to their profession or those who are mentally disabled and are unemployable?
Also, what about people who've built up an HSA but their medical costs far exceed what they've contributed and are elective procedures counted? I know someone saddled with a $75k helicopter flight to a hospital because it was deemed elective (they were rescued in the mountains, no other form of transport was available).
How are the elderly treated if they are no longer viable in the work force? We keep the same Medicaid system run by the government or would that be something else?

I take it you have never been in upper management in a large company or you would know this.
My employer pays 90% of our premiums. My company offers a "Cadillac" plan , thank god Trump got rid of the Obamacare tax on Cadillac plans.
The employees do not contribute enough to help pay for another employee in any meaningful way. My company eats the costs.
I pay $223 a month or $2676 a year for my family's healthcare (3 people). Our cost is based on family size not wallet size like government plans would be.
My company pays the rest $24,084 for my family's yearly premiums.
EVERY employee from a custodian to the president gets this healthcare.
Healthcare is a benefit provided by my employer in lieu of wages.

The first group is likely going to get Medicare via the Social Security disability system. The latter group is almost certainly eligible for Medicaid.

Medicare and Medicaid would remain intact. Social security remains intact. Insurance remains intact. The HSA would only be first means of payment. When depleted, insurance takes over.
The Requirement of the HSA as payment first would reduce the burden on Medicaid and Medicare though. And the healthy people who have large HSAs at retirement where it gains on investment would reduce Medicare burden also. They have the same right to Medicare, the HSA is just used first. When it’s depleted, Medicare kicks in immediately.
