This isn't Hamas or AQ, so this happening on some grand scale is unlikely.
But the reality is that destroying the infrastructure for Iran means a pretty large swath of pretty normal people suffer.
It seems increasingly apparent to me that Trump was sold on the idea that leverage from within would have mounted considerably by now. Our IC? Israel? Probably both TBH. So now we're left trying to manufacture it by pushing 'innocents' to the brink.

I dont like his messaging and I dont want to set a precedent that civilians or infrastructure are fair game because if/when terrorists do the same to us we will have no room to argue they've gone too far, nor do I think that doing any of that will win us this war or achieve regime change.
Sending out a tweet that says hes going to end their civilization makes us look as unhinged as the mullahs we are fighting so while I can accept there might be some minimal negotiating benefit I'd argue it costs us far more in terms of brand and perceived stability with our other more important issues like trade deals to be a net benefit to us.

That's not grand scale suicide, for one
Second, Iran has largely outsourced those things for about 45 years now
I don't think Iran is beyond playing dirty, but I the idea that Iran as a whole is accurately portrayed through the lens of revolutionary zeal just isn't realistic
