Someone had to cross domain that stuff manually.
As someone who has actually worked with messaging systems that handled classified information, I would confirm that's true. There had been times things would get kicked because caveats were not set right and had to send it back to the originator. As you said, someone would have to intentionally edit messages for such information to end up on an unclas system.

And right there Comey had a perfect opportunity to state that headers were intentionally removed, if that had been the case. Comey said no such thing.
