Sull, here is an option for you: fake it:

This image below is the Front and Back of the Covid vaccination record form used by every health agency administering shots across the country. It's what I received when I got the shots. I got one card with my first shot, and on the back is a reminder date filled in with a date for three weeks later. Be sure the month of March (if you do this) is 03, not 3. This is the color: DARK GRAY/BLUE used to fill in the blanks.

If you do this, the size of the card is 4.25" x 3.25" and is a light, semi-gloss card stock, not printer paper.

Under Product Name/Manufacturer here is the information to put in the rectangle: Pfizer-BioNTech

Under Lot Number: EN0000 is the format. Use any number combination to replace the 0000.

For Healthcare Professional or Clinic Site: use any County or Parish System, for example: Bossier Parish Public Health or Harris County Public Health

So that's the first card. On the first card, write in your Last name, First name, Middle initial, date of birth and leave the Patient number blank.

On the second card, do exactly as above, except do not include a Lot number and do not fill in anything on the back. Be sure it goes in the 2nd shot line.

While fake, it would give you the "proof" you need to go see your mother if you are adamant about not getting vaccinated.

Why not just fill in the 2nd Dose on the original card?

Because they hand you the first card when you get the first dose, and the second card is handed to you when you get the second dose.

I am an older mother myself, and I know what my answer would be to my children. I would do anything to keep them safe, anything.
