Based on what has been seen so far...either of his two ideas are possibly correct. Certainly wasn't the planes fault given it's approach and distance to run way.
It's either the helicopter or ATCs fault or a combo of both. Regardless this does appear to have been preventable and most NTSB investigations support that in many cases
