I would more or less fall in line with the UpToDate recommendation.
Funny thing is that's pretty much a solid summary of my feelings on it.
If you're hospitalized and worsening, I would consider it if you didn't have one of the many contraindications to it.

Let's just get this straight now:
Nothing is going to knock the mortality down to nothing. There are going to be bad outcomes regardless of what's done. When this shows any evidence of bettering outcomes, even in a high-risk of bias, poorly designed review, I'll be all over it. It's not even there. Reducing detectable virus in the nasal cavity means very little with regards to how the patients on it will do.
Numbers/data are one thing.
Lives saved are another.

The data mentions Jack shite about how they felt or how sick they were to begin with.
I read a lot of posts in this thread to seem as if people are thinking, "these are 26 people that would have died that now have undetectable viral loads"
In reality, viral load is an essentially meaningless stat. Outcomes are far more important than that.
It does show some promise. But you're exactly right- a study this small could be almost purely coincidence. When more numbers come out, you'll see institutional protocols and professional societies endorse it.
I can promise you that they aren't behind the times on the data.
The data sucks.

I think a frustration, at least for me, is that “we don’t know” isn’t being followed up with “but we are currently doing X,Y, and Z to learn more.” Right now everyone is just being told to stay home without any sort of timeline. That is not sustainable. Even if they were to say that studies are being done, but it will take 4 months at the earliest. That is almost as unsustainable as what we have now, but it’s at least an answer that people can begin to process. The vibe a lot of people who are presented as experts give off is that it’s just bullshite and they aren’t really interested in pursuing it. Just give us something.

I can assure you that this combo of drugs is effective, is currently in use, and production has been ramped up.
