I have a suggestion.
Excuse me nurse, I am likely not going to make it. My doc tells me I need to be on hospice and I have never felt a set of fake breasts. As yours are obviously real and perfect, could you recommend someone that might be willing to help me out?
The bolded part is very important.
In my experience the % of nurses under the age of 40 with fake tits is very high.
To answer your Q: No one makes them better than the creator. Saline is better than the older silicone but some of the newer gels are approaching reality in texture.
