I'm still contemplating this EDC, but most of the reviews I read are skinny dudes carrying appendix. I'm of the 3:30 - 4 o clock size, not sure if this or the X or the regular 365 would be best.
ETA: The issue is you can't try them on in the store with a holster, at least not any store I've been to.

I'm not a skinny dude and I don't carry appendix. I carry my Sig P365 very comfortably at 4-430 w/ the Ronin Leather "Okami" in the above picture. One nice feature about the "Okami" is the ability to easily move the holster some w/o having to take it off. I'f I'm driving, I simply loosen the belt, undo the forward snap, hove the holster to 3 o'clock my hip and tighten everything back up.

Glock 26 with G19 mags, get 15+1 capacity but nearly an inch shorter in length. Much more comfortable to shoot than the ultra slim 9's, no concerns about reliability and durability either.

Depending on clothing/season, an M&P 2.0 or a P365. Or pocket carry an LCP max.

Anything you're able to comfortably carry, can shoot accurately, in a caliber you can reasonably trust to work if you do your part.
Because I've had it awhile: a P64 in 9 Makarov, in an Outbags IWB (or pocket). It's about the size of a Walther PPK, in a slightly hotter rd than 380. I have the 18lb mainspring from Wolff, which brings the DA pull from VERY STOUT to reasonable, and the steel frame soaks up recoil.

For the Shadow Systems CR920XP I did the 200 rounds break in they recommend with 124 gr CCI ball & Gold Dots. Zero malfunctions. Dead on accurate with sights & Holosun 507k. Streamlight TLR-7. Weight balances perfectly & hardly any recoil at all. I shot a magazine as fast as I thought I could accurately hit A-zone at 10 yards & they were all about baseball size area in A-zone. All I need now is my Armzmen holster to arrive in a few days. Big thumbs up from me.

Springfield XDS .45. I had the XD 9 and it was a great pistol just a little too big to fully hide so I did some swapping and got the XDS.

Thanks. What I'm contemplating is carrying the xl vs the x at 4 o clock. Would the extra barrel length be an issue.

I honestly don't know. Whether or not it works for you can depend on a number things, including how the gun/holster/belt fit you body and your individual comfort leve are the first things that come to mind. lMy P365 is the original model and it's the only P365 I've carried. The key is having a nice holster and gun belt. I like my holsters to have a 15 degree cant. When I first started carrying concealed, I frequently carried a Commander 1911 in 45 ACP using an IWB holster. I don't remember the 4 1/4" barrel being a problem, but the all steel gun was heavy. If you know someone w/ the sizes you're considering, try to look at and carry them.

Your question is understandable because the cylinders are fat.
But it can work.
I carried a Colt Detective Special for a few years. I had the hammer spur bobbed so it would not catch on clothing. Ankle rig, 4 o'clock hold, (inside or outside the waistband) shoulder rig or even in a pocket. But that was many years ago before semi-autos became so dominant.
And it was more accurate than many people claim.
But it can work.

I have a Compact Dagger with MOS and really like it. I was a Glock instructor and spent a lot of time on gun ranges with them. Carried a Glock 27 for many years and it is a favorite, but the G-43 is so convenient and concealable... Sadly, as I have aged, my eyes appreciate the MOS for accurate shooting, so I went the PSA route. It was a bit stiff when received and there were a few moments of concern before I mounted the sight and began firing. Feels great in my hand and was every bit as accurate as any of the numerous Glocks I have shot.
Also very reliable. I hand it out to shooters at a local range for random shooters to try with whatever crap ammo
I have been wanting a Dagger Micro X-1 but they only recently became available and ones on the way.

When Smith came out with the model 60 way back when, the demand for it from law enforcement and all the way up to Judges as a low/no maintenance back up on an ankle holster was crazy.
Hard to find, waiting lists, you name it.
Smith could not make them fast enough.
Times change and is nice knowing you have 10, 12, 15 maybe 17 in a pistol not much larger than a 5 shot model 60
