Hmm...Got cup checked during a field exercise when I was in the Army that had me doubled over. Or that one time I was laying in bed and my then 2 year old son decided stomp on my crotch. Both had been a while but equally as bad.
Very good friend of mine lettered 3 years of HS ball playing shortstop and always wore a cup. Even when we played softball later in our late 20s. He then transitioned to third and I played catcher. Lobbed the ball while he wasn't paying attention and nailed him right in the nuts. Lucky for him he was wearing a cup.

5th or 6th grade, had a BMX bike but didn’t really do more than popping wheelies off homemade ramps. Tried to be clever one day, feet slipped on pedals, and fell into the frame of the bike. Had to limp home pushing a bike because sitting down was too painful.

Once took a kick to the groin in a street fight. Missed my balls but caught me dead on the head and gave that one-eyed SOB a black eye that lasted for a week. Hurt like hell to pee.

Was playing paintball with my cousins and one of them was messing with gun point blank and shot me in balls. Took me 2 hours to muster the courage to feel if they were in a million pieces.

My cousin when I was 12 or so came out of nowhere and flicked me as hard as he could. He squared up my left nut with his knuckle. One in a million shot. I blacked out for a minute and he was laughing until he saw how bad off I was. And then his dad kicked his arse. Which he did often. Which sucked.
Worst one I ever delivered was at camp. We were goofing off pecking each other with a hacky sack of all things. I came around the corner of an open door room and fired a shot right in this kids nuts. He had to leave camp bc of the swelling. I felt so bad for the kid. Alas.
