Poon or Strange was gonna be my answer. Also, death of a child. Many people never get passed that.

Divorce is over generalized here. A divorce in your mid to late 30's or even early 40's without kids isn't bad. When kids are involved, sure it's worse, but you all act like its a death sentence. To be honest, more often than not, people move on and lead successful lives with a new partner or by themselves. Addiction or a felony conviction is way worse than a mere divorce.

assuming the people don't own much or are on essentially equal economic standing? sure
if one or both of those things do exist, you can be put on your arse. it's basically like going into major debt
*ETA: this is ignoring the emotional toll and potential legal fees, which is like another set of massive debt

lawyer i know just found out his 45-year old wife is preggo. he's in his 60s
That last part is rough.
That's one of the reasons I avoid a significant age difference.

Drug addiction and alcoholism maybe? Depression
I've got two of the three, and my career is going along swimmingly. Five years until retirement.

divorce or a child then separation really get people. It can cut pay in half and totally change the way of life.
Drug addiction is bad too, I just have a hard time feeling bad for people who go that route.

The people I detested the most were the ones who made retirement their major goal. All their decisions were based on what would protect their retirement and not on what was best for the company/users.
They would never take a chance unless they already had someone to blame it on.

It's fairly common and manifests itself in various ways. Some just keep it together better than others. The self treatment usually gets blamed but at the root is boredom, or restlessness

i can definitely see how that would lead a person to making mistakes at work or just generally not trying (and the after effects like not advancing, etc). lots of research is coming out that happiness is not doing what you love, but developing/mastering skills and then doing it all over again

I would say divorce/being deceived by spouse.
I had two aunts (one on each side of the family) who had great lives that fell apart.
One aunt had a huge ranch house, tons of land, husband ran a successful business, they had plenty of money. Aunt was a nurse too. Turned out her hubby was double-billing clients or something and got caught. Everything fell apart. She ended up in an inpatient mental health facility after trying to end her life, lost her nursing license.
Other aunt was married to a computer programmer back in the 90s. He was a whiz, made tons of money. They lived a good life. My cousins had everything 2 kids could want.
Then it turned out he had gotten in trouble for drinking vodka on the overnight shift. So to retaliate, he stole computers. Got fired, and he abandoned my aunt. She went wild, started clubbing, going from one guy to the next. Cousins grew up, one overdosed and died, the other is in and out of prison. The hubby died last year, he ended up addicted to cocaine.

Depends on how severe the term "derail" is in context.
My life has been derailed, but I wouldn't say that it's destroyed. I realized that my career wasn't for me. I'm still treading water trying to figure my life out at 31 years old.
I would imagine divorce could be the worst depending on the circumstances. Buddy of mine got a divorce and sees his kids one weekend every two weeks. His wife initiated the divorce, and he didn't see it coming. I really can't imagine the pain he's going through slogging to work every day knowing that everything he thought he was building was destroyed by a signature.
