I'm taking the position that when the OP finally had enough and looked for a waiter or hostess, there were none around.
I've seen this happen at 1 star to 5 star restaurants.

Exactly. 5 star restaurants have a team of waitstaff from, water boys, hosttest, servers, waiters, waitresses, maître d', bread crumb scoopers, plenty of staff around to get someone’s attention.
