Underwear sizing help

I’m a pretty slim, fit guy. I’m somewhere around 5’11 150 lbs. The problem is my underwear doesn’t fit right. I put on a medium and it’s way too tight around my package, even though it would otherwise fit my waist and legs fine. I put on a large and it’s still a tight around my package, but I could live with it if they would stay on my waist. Do tailors do underwear or are there any made for this body type? I need the “package area” of an XL and everything else a medium. Thanks in advance.
