Those drain pans need to be checked and tested yearly too. Some loose insulation or a mud wasp nest blocking it up renders it nearly as useless as not having one. I check mine when I check the ac unit's condensate pan.

Maybe we have been lucky and I’m sure it varies by water quality but we haven’t had to flush ours and it’s been around 8 years

The way to go, but I do not like any plumbing in the attic. Garage is best imo, especially if you already have a sink with water available.

The ventilation can be a bitch. Also horizontal rains can shut you down for hours if not

Most plumbers and builders do not run overflow systems to the sewer because there is no indication for the homeowner that a leak is occurring.

Most run them to a porch or common area outside so that the homeowner knows when they see water in that area that it is not normal and something needs to be checked.

LIttle Update:

Talked to a plumbing supply guy. he was kind of discouraging tankless electric due to the need for more amps that probably isn't already there.

Talked about the tanks and different sizes, he also suggested that they sell boosters that can somehow help put out like 30% more hot water output that goes ontop of the tank he said. Think about a $240 product.
