Going to a game in Baton Rouge, I was given a room key, and upon opening the door saw luggage and clothing on the beds. Same deal, they gave me an occupied room.
In my decade plus of constantly traveling, I only ever had one incident. I checked into a hotel room for a 2 night stay, received my key, went to the room in question. Opened the door to find a man & a woman in the middle of a love session. After a surprised few seconds of gawking (they were turned away from the door) I closed the door and asked front desk for a different room. I'm not sure if that happy couple ever realized I'd opened the door.
Edit: and I just remembered exactly where it was, too. The Clover, located near Notre Dame campus in South Bend, IN.

Years ago my Dad and I took a motorcycle trip down to Grand Isle, LA. He wanted to go down there because that's where they used to go on vacation when he was a kid back in the 30's and 40's. That place was so trashy looking my Dad insisted we move a dresser drawer in front of the door that night.

Every hotel/motel I've stayed in had those bar type safety locks on the inside of the door. No need to stack up empty beer cans next to the door.

Im sure that's not how key cards operate. That key has to be made by a specific key machine, through a program, at the front desk, for the key to work. Saying I'm in the hotel business, and just upgraded our hotel locks, this is the first time I've heard about this.
