How do you tell the difference between a fracture and a sprain? I had a gnarly bicycle wreck and landed on my arm. I can rotate my shoulder, move my fingers, and bend my lower arm at my elbow. I cannot lift my arm to the side or in front of me. There was never any swelling.

My fracture didn't hurt as much as a sprain

I am really not in pain unless I suddenly move.

You should go see someone and get an X-ray.

You should go get that checked out

That could be any number of things including a separated shoulder from your description. Was the majority of the impact to the shoulder? Do you have constant pain or only on movement? No swelling at all?

Impact was mainly on the left upper arm. Only pain when moving. Never any swelling.

And this happened when?
ETA. The fact you are up writing about this at whatever the hell time it is means you should go get seen. BUT I would skip the special hell of the ER at night at this point and go tomorrow. That being said, this advice is worth what you paid for it.
RICE Rest, Ice it, Compression (in this case sling it to avoid movement) Elevation (doesn't really apply here). So, for a shoulder, the classic sling coupled with a bag of frozen peas on the shoulder and some Acetaminophen would be my middle of the night recommendation.

Swelling is not a ruling in or out factor in regards to fractures. You need an X-ray.

Saturday. I also cannot put weight on it. I thought because of the minimal pain (unless moving) and swelling, that movement (lift ability) would come back. It really has not.

I am starting to believe that you are correct.

Sounds like a fracture to me. Definitely head over and get an xray

Regardless you need it checked out.
Fooling around and getting a frozen shoulder is no fun.

Weight bearing is the classic in the field determining factor for fracture actually. For field use, if a guy could walk on the leg/ankle we let them. If they couldn't bear the weight, off to the X-ray machine they went.

Thanks guys. I will go to the ER tomorrow. BR General or Our Lady of the Lake?

Chem the reason I believe it is fractured is that you are unable to move your arm to certain postions. When I broke my hand I could only squeeze to a certain point. Until my hand was healed with a reset and a cast. After that I had my full squeeze ability back. Just my .02

I really appreciate your two cents. I will get it looked at.

I'm in orthopedics.
Sounds like a rotator cuff.
You need the X-ray to rule out a couple things:
#1. Fracture (but it would probably hurt to externally rotate your arm too)
#2. AC Joint separation
X-ray can tell both of these.
The other possibility is a labral tear. If your arm popped out of socket or 'almost' popped out, you might have torn the labrum.
I'm betting rotator cuff based on your description and the other limited info I have access to.
Good luck.
