It’s not uncommon for a lot of health conditions to be undiagnosed and “discovered” when kids are school aged.

In my experience, these are done in order to assist the first responders attending an emergency. Once your kid gets to the ER, they’ll be able to pull his records up anyway. But God forbid your kid is allergic to something & the EMTs give it to him on the ride to the hospital.
I’m not a doctor or a lawyer, just a helicopter parent.
If you’re unsure, ask someone in the school office what they know about Hippa. If they don’t take the hint and give you a damn good answer to that question, don’t sign it.

The other thing this allows is for the treating physician to talk with the school about what they are doing in your absence.

So the only way this works is if the school gathers medical information for all of the students and creates a medical file for each kid...then they would need to transport these medical records on all of the field trips and extracurricular activities...that's a bit much

They probably don't even read and question what they are sending out to you. They most likely printed some form off the internet edited it with their school logo.

I was thinking more along the lines of having information on file or accessible as needed. So if a kid has a seizure or something, they’re able to pull any relevant info and be able to tell EMS upon arrival.

Most kids don’t have an complicated medical record. Therefore for the kids who have one that is complicated, it would be wise to make it available for a field trip (ie seizures, allergies, etc). The gray area is where the kids are on ADHD meds or something, and the parents don’t want the school to know.
