Basically like gthog61 said. If the roadway that breaks the median has a stripe, or two-lane traffic, then you turn into "your lane" while crossing the median.
In every other circumstance, you make your turn ASAP. The alternative, where two left-turning vehicles cross each other's paths inside of an intersection, is asinine.

In America we drive on the right side.
What happens if someone is coming toward you on that side street?
You are in their lane.
Also the answer to your long line of cars problem is don't block the intersection while you are waiting your turn.

Sometimes the side street has a red light and you have a green.
In either circumstance, the "drive on the right" rule falls by the wayside, unless there is striping indicating otherwise.

No it’s not. It’s only asinine if there’s a line of cars in a turning lane behind one of them. In a regular residential area where that isn’t the case and there isn’t a turning lane and just a median area for people to turn, yes it absolutely makes sense for the two cars to pass each other and go to the side where they can see oncoming traffic. It drives me insane when people don’t follow that rule.

Not to mention, most medians provide a turn around at a place where someone could be turning left onto a street or driveway and not doing a u-turn.

This is correct. If there’s a red light at the intersection, you take the closest path when turning left unless there’s a yellow stripe in the middle, like you often see in NOLA.
If there’s no side street none of this matters because it’s not an intersection.
If there is a median and an intersection, you are supposed to get on the right hand side. There’s an intersection like this near my office and it drives me crazy when people do it wrong.
Usually it’s not people turning left from the oncoming lane that cause a problem. It’s people turning left from the side street. Instead of staying in their lane when they pull into the median area, they pull in diagonally and block the intersection.
So now drivers going both ways on the main road can’t pull into the median area, so they stop in the left lane. Drivers behind them get pissed off and start whipping around them, but the jackass parked in the intersection can’t see them because of the traffic jam he’s caused. That’s the kind of shite that causes wrecks.
