Is this the cause of FM or the effects of FM? Do they have low serotonin levels due to FM, or does the severe depression due to low serotonin levels cause physical pain? Is the increase in substance P in the CSF a cause, or is just the effect? Would an intrathecal tabasco pump help deplete the substance P and give them relief?

No I’m not a doctor and yes I know that’s anecdotal.

i mean... anecdotal experiences are legitimate to an extent and can reveal truths about a patients behavioral patterns. however, anecdotes are also influenced by personal perspective.
... and, i will be one quite soon just not the kind anyone cares about.

Given the altered allele frequencies among candidate genes, including some mu opioid receptor genes, I would wager these are effects and not causes. Given that knowledge, my inclination is to regard FM as a group of disorders that include dysregulation of pain processing with cognitive and musculoskeletal sequelae, with some inflammatory markers present, and some HPA axis and NT dysfunction. I don't think a specific, distinct pathophysiology has been worked out yet, but the genetic data is promising. Given my education was strictly on the molecular aspects of this disorder, I couldn't suggest any new treatment course.

Any studies on CRPS (ie RSD/causalgia) in regards to serotonin/CSF substance P/etc?

Was just wondering if findings were similar. I’m thinking immobility—>spasms—>pain—>further immobility—>cycle repeats itself.

ETA:
CRPS obviously has an autonomic component that is abberantly unregulated. But it is also treated with steroids, unlike FM.

I think a likely process is nonpainful stimuli regarded as painful, with an inflammatory response followed by/coincides with dysregulation of cellular muscle function, which magnifies the inflammatory response. But I have to more in the literature.
