Olin on the Westlake side of Calcasieu River used to make Phosgene and there were several times that drivers on I-10 were gassed.

They removed a PSV when there was still a N2 flush going on in the line. Contractor was suffocated by N2

Friend of mine was instrumentation vendor to plants around Houston, got hit with phosgene, survived, but caused progressive lung damage that killed him about 10 years later.

Yep. Probably Zachry. BASF ran Turner out around 2014 and brought in Z but a lot of the workers just changed hats from Turner to Z

This is odd. I don’t work with the chemicals they use so I don’t know their procedures, but I don’t understand a N2 flush on a line to remove a PSV. I take this to mean that the stuff in the line was so bad that ops wanted to blow down the line on each side of the PSV to a closed system. If so it would have to be nitrogen that’s pressure rated below the PSV rating so you don’t screw it up. Just seems like a lot would be going on right there for someone to not know that it wasn’t ready to be touched. Biggest red flag would be the valves on either side being open and obviously not LOTO’d.
I’m just wondering how they were given the green light from ops to touch that equipment without it being prepped and ready. At our site we prep equipment, LOTO, get another guy to verify LOTO, communicate with mechanical on job scope, walk the job out and then sign the permit and THEN give them the equipment.

Unfortunately people get complacent and tired and just overlook or skip steps because they’ve done it 100 times.

This is terrible. I sure hope he died of Nitrogen suffocation and not phosgene exposure.

I’m not saying that’s the case here but in just speaking to my wife and friends who work at this plant it’s really my scary how close y’all get to accidents
One story that scares me is someone who was looking at a steam valve when the pipe leading to it blowout. Thankfully it busted on the side opposite of the tech. Had it let loose on the other side he would have been decapitated instantly

I hauled MDI out of there for a few years. Tempur pedic was a big account. Bridgestone. Foam companies. Fresenius.

As is CO which is also used in the process of MDI and TDI.

Not necessarily. A small dose exposure of phosgene CAN kill you, in the right concentration and it really depends on how much you truly inhale deep into your lungs. Not easy to measure that ,in hindsight.

Correct N2 is used in every plant where phosgene is in only a few. N2 is an asphyxiant vs phosgene being toxic.

Absolute do hate to see this. I've got 35 years+ in the industry. Terrible thing for this person and his family.
