Residents and fellows are still MD's.
Plus primary care and E&M based specialties just aren't that lucrative.
I would think those factors drag down the average.
ETA: And the survey is probably only using their primary practice data. Not all their ancillary investments.

Probably the same reason accountants never make these lists. They always lump in the entry level accountants with CPAs & Directors of Accounting and/or Finance since they are all technically accountants.
