My wife has a 2008 Honda accord v6 with 208k miles. In last year, we have replaced the spark plugs and ignition coils because the check engine light keeps flashing and is real rough. We keep replacing the ones that the obd2 scanner pulls up. What would be another cause for it to keep misfiring?

How many times have you replaced them and at what mileage intervals?

Did that obd2 scan tool suggest EGR?

Throughout the past year, we changed all of them changing two at a time that the obd2 would say that was bad. It happens about every 2-3 months. Bank 3 and bank 6 I know we have changed twice already. Cables are fine and it did not suggest to change the EGR.

Put a piece of black tape over the light. Problem solved.

Sounds like the VCM is fouling your plugs.
Get a VCM Muzzler.
It keeps all of the cylinders firing all of the time.
You might lose 1-2mpg.
I have it on my Odyssey, where this is a very common problem.

Not to question your intelligence, but did you check to see that there actually was a misfire at each location indicated, or did you just trust the sensor (OBD2 readout)?

Good suggestion to check the VCM.

Could be the piston return springs or muffler bearings. Hard to know without doing a complete teardown

With the VCM as stated combined with the oiling issue, a plug(s) intended to be firing when the VCM is active may be fouled sufficiently for the CPU to shut down the injector(s) to cause the lumpy running.
