My guess is that this is 1.3 million to shore up NO EMS. Hospitals have already written off long standing outstanding balances

Would agree with your assessment as it applies to the hospital based systems like Ochsner, etc. But private unaffiliated docs take losses on Medicaid.
This amounts to about 1/75 th of the funds or about 1.3 million. Maybe a payoff to Acadian.....does Acadian even operate in Orleans. I suspect this goes to EMS guys in particular.

They need to tighten up their operations if they do. They get in the neighborhood of $50 per office visit not counting any other billable services they perform during the visit.
I have a good doc and rarely does he ever spend more than 10 minutes with me on an office visit. That's $300 per hour.
Healthcare should be subscription based and not fee for service IMHO. Paul Ryan, BTW, attempted to move Medicare to this and both parties fought him as providers and drug companies and insurance companies screamed.

Government payers might make up revenue due to volume, but commercial/private insurances reimburse at a significantly higher rate. That 70% is on the higher end of what most healthcare systems see their revenue share at ( LINK)
That $50 reimbursement is usually correct for new patients. For an existing LA Medicaid patient, a ten minute visit would reimburse between $25-$30. You're describing a perfectly run clinic with no gaps between appointments, fully booked all the time, and with no patients missing appointments. That's an ideal world, but not what actually happens.
Do I think there needs to be an overhaul of the current system? Absolutely. Do I think a subscription-based product could be a reasonable option? Sure. But I think there are multiple parties to blame for why healthcare is where it's at right now, and the government options play a role in this.
