Looking at healthcare.gov it's:
2015 - $325 or 2% (whichever is higher)
2016 - $695 or 2.5% (whichever is higher)
You're gonna pay the percentage unless you're just poor as frick.

the reason that particular person's bill is going up by a particular dollar amount.

They fine you when you file your Federal Income Tax return. I guess if you don't file you're not subject to the fine.

frick, if Terd is right then I have to pay $1700 penalty this year.

they asked a question on the return if you have insurance (yes/no)
if you claim yes - you then need to prove you have it.
if you claim no, they give you a warning that they are taking $$ from your refund
if you claim yes - you then need to prove you have it.
