JAMA interviewed the head of an ICU unit in Lombardy on Youtube this morning.

He gives a rather sobering view on what they are facing.

They had 10k cases on Wednesday and now have 15k cases. Our definitions of 'going down' is very different.

I’m not following the numbers but does he mean there were 10k new cases one day then the next only 5k?

The percentage dead is given as a percentage of total dead. OK, I'm in one of the higher percentages. I'd really like to know what % of the cases in each age group are dead and what % are proven survivors (and then I can do the math to determine how many are sick).

No, that was total number of cases. As of now they have 27,980 cases.
