Possibly first used by Alanis Morissette for her 1995 song “Head over Feet”, although in the original context it seems to refer to a committed rather than a casual relationship.

I know you metioned "popularized" in your OP and this clarification, but somehow I doubt the phrase bad hair day was "coined" or originated in the 1990s.

We used to say my bad in 1982. Our center on high school bball team when he goofed up a play, "my bad". Errbody made fun of him because it was stupid then.
