I believe that you are being intellectually honest about the current state of inpatient mental healthcare. Healthy and stable discharge should always be the goal. I think that the fear that some are expressing here involves how that may change when large amounts of money are put on the table to address things, which currently isn’t the case. Money tends to greatly affect the motivations of people.

Firstly, streamlining mental health services should, in theory, save money, as the administrative costs are the places where there seems to be the largest disconnect between what services can be provided. At the same time, how do we deal with perverse incentives which appear with even tiny amounts of money.

I was involved in a very small group who was putting on events through a hospital foundation, with the aim of bringing families who have children suffering from various medical and mental conditions together so they could network among themselves. The point of this was an attempt to create a sense of community and build upon it so that resources could be shared inside the community. The first year we spent only 2/5ths of our allotted budget and put on an event for 30 families. The next year, with only a marginal increase in what we spent, we held an event for 50 families, with many return participants. The last year, the hospital foundation was impressed by an outside organization who promised lots of things, but instead of networking, which was the original intent of the event, you had paid weekend getaways which included that organization's staff, with only 8 families invited to the event, and no intermingling allowed between the families. They spent 3 times what we spent for what amounted to a vacation for their staff.

I don't know of a structural way of dealing with chronic diseases, in terms of maximizing what is best for patients and families, in a directly profit-driven system, or where there may be perverse incentives at play. Ultimately, being as parsimonious as we were meant that we were unprepared and completely blindsided by the foundation's decision, in such a way that there wasn't a net benefit in the long-term in being responsible with the money, especially for the families that we wanted to serve.

Nah I'm amazing in stressful situations. And it's weird how you don't seem to address your nonsensical either/or scenario which prompted my response. It's only the usual chuddish grandstanding, i.e., 'hur dur ur so mad' and 'I was just joking bro.'
