The type of people who get put on holds are generally not dangerous to this degree. Most are schizos or bipolar and usually are more at risk to themselves or family.
No, these killers are almost never seriously mentally ill. Most of them do, however, have personality disorders (which technically are mental illnesses since they are in the DSM). Narcissism and anti-social personality are the order of the day and those are not really disorders that can be treated with medications.
There are exceptions, of course. John Hinckley was definitely delusional (likely schizophrenic). Charles Manson was probably on the schizo spectrum as well. But most of these mass shooters and serial killer types are perfectly normal by all appearances (Bundy, Gacey, BTK were all pillars of their communities). Many are loners, but there's plenty of loners who don't kill people.
I think it's more of a pre-frontal cortex lack of impulse control. They get extremely angry and cannot control it.
