Not necessarily. For many it is just standard expected behavior for their culture.

I typically don’t, but if I’m coming in from outside just to grab something inside and go back out, I’m not taking my shoes off for that. Otherwise, yes I take them off if I’m staying inside.

I don't request it, but I have no issue with those who do. Now, I do take issue with those who demand it. Unless you alerted your guests to that impending demand beforehand, you are potentially exposing them to embarrassment if their feet/socks/etc aren't in a state they would want displayed. The Japanese fix that problem by having a row of sandals/slippers in the doorway for guests to use. Unless you are willing to offer the same courtesy, swallow it and be a good host.

Never when I'm going to stay. I don't take them off if I'm about to leave again.

Yes, and I don’t go to people’s homes who expect you to take off your shoes, it’s weird.

Using culture is not an excuse for weird behavior. There is a lot of things in culture to be lost and forgotten for the betterment of civilization.

The need to remove one's shoes in today's 1st World societies does not make logical sense. We are not the same people we used to be and it is ok to let go.

My wife's grandfather was weird about that. It was such a pain in the arse when you were going inside and outside at family get togethers.

I'm not sure if dive bar bathrooms have gotten any cleaner in recent times.

I wear shoes or slippers. I don't like walking barefoot. I've never been to a house where I was asked to remove my shoes. There are mats at all the doors if you need to wipe off your shoes. There's also no entrance which would cause muddy shoes other than perhaps a wet bottom of a shoe if it's rained.

Yes.

Hardwood floors and I'm not taking my shoes on and off as I go inside and out.
