One of my buddys wife always comes in for the hug/ cheek kiss combo. It is weird to me. I do not kiss her on the cheek. When she greets my wife, she kisses my wifes cheek.

it's definitely common in the new orleans area. much more so than anywhere else I've seen.

I shake their hand like you're supposed to shake a woman's hand (cup her hand in your palm). It freaks out the ones that aren't from here

No. Like OP suggests I could see it making some people uncomfortable. Among my family a hearty hug might be expected. People would think that I was trying to act fancy or just downright queer if I went in for a kiss on the cheek.

I'm talking about women. You don't generally hug men either. You shake their hands. A kiss on the cheek to my one of my aunts or one of my female cousins and they would likely think that I was acting a little fruity, like one of those fashion show queers.
