But really, why should health insurance companies charge identical premiums to people regardless of overall health?
Life insurance companies have been sizing people up, so to speak, forever. Many will send a nurse to your house to weigh you, take your bp, and draw blood. Your rate is dependent on overall health and risk.
Car insurance works the same way. Do you have a bunch of tickets and accidents on your record? If yes, your rates will most definitely be higher.

To be fair, there are a couple companies I've worked for that had health insurance policies that would increase slightly if you used any tobacco products.
With that said, smoking presents a whole host of heath issues that would cost more over time. I agree with your assessment, why the hell shouldn't companies do this instead of making it political now? Oh yeah, because this grabs headlines so they can signal all the virtues.
It's no different when CVS announced they would stop selling tobacco products because they're concerned with their customers health. Yet they still sell junk food, sugared soda, alcohol, and a whole lot of other things that contribute to obesity.

They already do for tobacco users. For folks that are unfit, they'll generally offer wellness programs designed to get people active and in return will reduce their health risks and their premiums.
There will be exceptions for those who are unable to get the vaccine due to medical or religious reasons in the form of accommodations. The stickler is if they decide any accommodations they have to make for the unvaccinated will result in financial hardship for the company, then your arse is grass.

COVID should eliminate all arguments that the right is the side of tyrants. I know it won’t because the left doesn’t think for themselves, but it should.

My company does do this for smokers (additional annual surcharge). There is an annual health screening required before signing up for insurance, and if they find traces of nicotine in your blood work they charge you the smoker surcharge. If you refuse, they charge you for it anyways. If your BMI or waist circumference is over a certain level they make you meet with a health specialist or pay a fine.
