I once had a $21,000 helicopter ride, for a week's vacation in ICU..insurance covered it but I've often wondered if not, would they have tried to come against my car or a house, assets or something? (was in louisiana). Or would there be some settlement figure. Etc

They can send it to a collections agency who could then decide to take it to court and get a court order for garnishment if you don't pay up.

They'd just sell your debt to a collections agency.
