Hot, but not scalding hot.
Hot water will cause an immediate sensation of either intense itching or relief (I've heard it described both ways). Afterwards, you will not itch for about 8 hours after which you will need to repeat the application of hot water.
Start by putting the affected area under a tap and turning on both hot and cold. Slowly decrease the cold water until it is as hot as you can tolerate. Stop once the above-described sensation goes away.
This may also shorten, by a little, the duration of time before you stop having symptoms altogether.

It cannot be overstated how effective this treatment is for severe poison ivy rash relief.
Several years ago, I was clearing some bamboo from my property and got my bare arm tangled up in a bunch of poison ivy. It was bad, bad. My dermatologist suggested this treatment and it was shocking how well it worked.
Also, if you hit the perfect temp, your body will dump a flood of endorphins and you'll get sort of high off it. It was weird and wild stuff.

Nope. Misery for days. A medical induced coma would have been great when I got a bad case about 20 years ago.
