The Sweet Hereafter
Nothing really comes close to that. It feels like your happiness is being sandblasted for 2 hours straight. And after the sandblasting, it's laid in a coffin which is burned and then buried in a deep ocean trench.

With Marley and Me, I thought the movie was horrible up until the ending. I knew I'd tear up, so I stared at the wall beside the screen till the movie was over.
It worked, but then all I could think about was my dog dying the whole way home.
:ETA:
He'll never die!

Gallipoli is one of the most heartbreaking films of all time. Wit, the adapted play starring Emma Thompson, was pretty sad.
About Schmidt is another very sad film.
The Kathy Bates full-frontal nude scene scarred me for life.
But NONE of them get me like Dr. Zhivago, in so many ways on so many levels. It's a masterpiece.

There was an ABC television movie of the week in the 80's Called "Bill", starring Mickey Rooney. It was terribly sad. I cried for days on end.

I've only read page one of this thread, but I'll go with Old Yeller as a top one for me. Also, there was a thread a year or two ago about animated movies that were really good, and someone mentioned Grave of the Fireflies and how good it was, and how it was one of teh saddest movies they had ever seen. I remember I watched about a 6 minute clip from it and was depressed and sad as hell.
Still never saw the whole thing though.

"Where the Red Fern Grows."
This movie is almost impossible to get through without at least a tear being shed. As powerfully sad as the movie is, the book is even sadder.
The grave scene with Jenny as well as when Gump meets his son for the first time always get to me a little bit.

Cannot believe it took 3 pages to mention this movie. This is the answer.
Good call on Simon Birch as well.

I'm sticking with Old Yeller.
dude had to kill his own dog which had saved his family several times because the dog contracted rabies while in the act of saving the family.
a boy and his dog ....
