Mine is definitely Angels & Demons. They changed up a ton of the plot and eliminated the 4th most important character entirely.

Lords of Discipline by Pat Conroy
Great book. Awful movie.

Worst in terms of quality of film or in terms of faithfulness to book?

I guess it could be either, but I was going more along the lines of faithfulness to the book

Pat Conroy's A Prince of Tides.
or maybe Eragon by Chris Paolini

I din't care much for the book, but the movie certainly was not faithful.

Without question Bonfire Of The Vanities. A satire on political correctness was turned into a monument to political correctness.
Classic moment -- Melanie Griffth reacts to driving through a black neighborhood: "Ah'm from the South, and ah'm beginnin' to not lahk this!"

Sum of All Fears
ETA: There are maybe 2 scenes in the entire movie that happened in the book, and even those are changed.

Valentine by Tom Savage probably wasn't a well known book but it's an awesome and suspenseful book and it was turned into a shitty slasher flick.

Timeline and Sphere by Micheal Crichton.
Two of my favorite books.
Extremely shitty movies.

My choice too - that was a good book and the movie really screwed it up.

Sphere...epic fail. Congo was also really bad, but Sphere is by far my fav Crichton book and the royally effed this up.
As for Sum of all Fears, i've never even watched it b/c it looked that bad.

Everything Stephen King ever wrote that was adapted to film except "Stand by Me", "The Shawshank Redemption", and "The Green Mile".
Oh, and "Cannery Row" by John Steinbeck wasn't anything like the book, but I liked the movie more than the book....

I think they tried to make the "The Dark is Rising" series into movies but the first one failed miserably. Loved those books as a kid.

Well considering the book sucks as well, I don't find it to be that bad of an adaptation. It's just a blatant ripoff of Star Wars and Lord of the Rings, with the Dragonriders of Pern mixed in.

A Passage to India
Book is sort of a classic if you like EM Forster but the movie is one of the worst films ever made.
Disclosure - Chrichton
Dune - terrible.

I am a huge James Lee Burke fan - Heaven's Prisoner was an absolute horrible movie with Alec Baldwin playing Dave Robicheaux.
Also a fan of Carl Hiaasen, "Striptease" was one hellaciously funny book (as are most of Carl's books)... but the movie was just not funny at all.
As for completely deviating from the book, I agree, "Sum of all Fears" was absolutely the worst movie for staying true to the book.
