Downfall is definitely up there. Mostly for the scene with Frau Goebbels and the kids.

Requiem for a Dream is #1 and it's not fricking close. At all. Some other choices are Marley and Me, Schindler's List, The Road, and most recently, Manchester by the Sea.

The River's Edge with Keanu Reeves and Crispin Glover. Old movie from the Eighties I think. Haven't seen it in a long time but I remember it being pretty dark.
